Please download our application form by clicking the image above or here.

Please fill in the form as far as you can. You will need at least two Godparents, who need to have been baptised. If any are not baptised, this can take place in the same service.

Please contact the parish office in the first instance and the minister will arrange a meeting with the parents.

Baptism is free but if the parents wish to have a baptism certificate there is a statutory fee. See Baptism Fees link below.

Would you please pass the payment preferably to “All Saints Church PCC” – to the Baptism Visitor, or post it to the church office.
